Background: Pubic related groin pain is a condition characterized by pain with palpation to the pubic symphysis or parasympheseal region. Etiology is not well understood, however it has been suggested to be an overuse injury resulting in pathology of the pubic symphysis and adjacent pubic bones. This injury is commonly seen in soccer, hockey, and long-distance athletes. Diagnostic criteria for groin pain are scarce, and what is available is often vague and inconsistent. Purpose: This report seeks to serve as a summation of current evidence on pubic related groin pain through the framework of a patient case study. Case Description: The patient was a 44-year-old marathon runner presenting with groin pain that began 6 months prior to the initial visit. After several visits with no clear diagnosis, his pain was reproduced with palpation of the pubic symphysis and the diagnosis of pubic related groin pain was made. Other conditions such as femoral acetabular impingement and inguinal related groin pain were ruled out to assist in making this diagnosis. As this is an overuse injury, his treatment was able to be targeted at a cessation of running with a focus on hip and core strengthening. Discussion: This case provides an example of the diagnostic process of groin pain. The patient had only one characteristic of pubic related groin pain, and through process of differential diagnosis other conditions were able to be ruled out. Future research should focus on continued delineation between the different groin pain conditions.
