Abstract

Background: Expert consensus suggests that same-day discharge (SDD) of patients undergoing trans arterial chemoembolization (TACE) is safe with well-established protocols, but the institution currently does not have protocols in place to assess for SDD which leads to the use of an observation bed, increased patient load for the admitting team, an increased risk of patients experiencing adverse hospital events, and an increased cost of stay. Purpose: The purpose of this DNP project was to implement a discharge checklist for patients undergoing TACE in IR that assesses the eligibility of same-day discharge, reducing postprocedural hospital stays in observation beds. Methods: Chart review was performed on patients who had completed TACE between August 1, 2025, and October 31, 2025, and checklists were distributed to nursing staff for these cases along with a nursing survey. This data was collected at the end of each case and analyzed. Findings: 100% of patients that were identified by the checklist as safe for same-day discharge, were able to discharge home same-day without incidence. The checklist was reported easy to use by nursing staff and a high level of satisfaction with the discharge process was identified. Discussion: By creating a system to identify and ensure safe discharge for select patients, this nurse-led initiative has prevented unnecessary overnight hospital observation leading to reduced utilization of hospital observation beds and decreased cost of care to the patient.
