Background: The prevalence of total hip arthroplasty (THA) is steadily increasing with an exponential increase in younger populations undergoing the procedure.Current physical therapy rehabilitation protocols are designed to return patients back to baseline activities of daily living and are typically discharged once these activities are achieved. There has been minimal research regarding higher intensity rehabilitation programs designed to return more active patients to higher-intensity activities. Thus, the purpose of this case study is to provide a clinical example of an active patient that benefited from more rigorous physical therapy and returned to more demanding activities. Case Description: The patient is a 40-year-old fitness instructor that had suffered from chronic hip pain that was progressively getting worse. An MRI confirmed a left CAM and Pincer lesion, left superior labral tear, and grade four OA. The patient underwent anterior approach THA and proceeded to attend physical therapy. Interventions: Physical therapy focused on improving baseline strength, range of motion (ROM), and functional mobility at the beginning of the rehabilitation protocol as do typical total hip arthroplasty protocols and once these were achieved progressed to higher-intensity exercises with a focus on return to higher-intensity activities. This portion of the rehabilitation included heavy lifting mechanics and plyometric exercises. Outcome Measures: The primary outcome measures included subjective pain report, ROM measurements, manual muscle testing (MMT) measurements, and the Lower Extremity Functional Scale questionnaire. Discussion: This case report demonstrates an example of a higher intensity rehabilitation protocol that aided in the return to higher-intensity activities.
