Background: Low back pain is the number one cause of disability globally. A recently published Clinical Practice Guideline states that there are numerous interventions a physical therapist can implement into a patient’s plan of care including patient education, exercise, and manual therapy techniques to reduce pain and disability caused by low back pain. It is also vital for a physical therapist to consider the patient’s biological, psychological, social, and emotional factors because they can impact their pain experience. With that in mind, the purpose of this case report is to explore the rehabilitation management of a complex patient with low back pain complicated by bilateral knee osteoarthritis and psychosocial factors. Evaluation: The patient is a female in her 60s with a chief complaint of low back pain following a car accident. Of note, she also experienced fear avoidance tendencies, anxiety while driving, and ongoing bilateral knee pain. The patient’s goal was to return to her daily activities pain free. Interventions: Throughout her rehabilitation management, the patient received a multimodal approach to treat her low back pain including patient education, exercise, manual therapy techniques, and modalities. The exercises prescribed to the patient targeted core stabilization and hip strengthening and were progressed as tolerated. Outcome: The measures used in this case to monitor progress were subjective pain ratings, objective information such as range of motion measurements, and the Oswestry Disability Index. By the end of her time in physical therapy, the patient reported feeling 99% pain free during functional activities and showed improved range of motion through her lumbar spine. Discussion: This case report highlights the individualized treatment of a patient’s low back pain and how interventions were adjusted to address the patient’s biopsychosocial factors.
