Background: Huntington’s disease is a disorder characterized by chorea, dystonia, and bradykinesia. Overall, there is a lack of quality research related to physical therapy interventions for Huntington’s disease. The LSVT BIG program trains people with Parkinson’s disease to use their bodies more normally and should be applied to other neurologic populations. Case Description: The patient was a 48-year-old male who had been diagnosed with Huntington’s disease six years prior to seeking physical therapy treatment. He had recently been having worsening symptoms of chorea and decreased balance control. He had recently experienced a fall in the shower and had significantly decreased his activity level due to this fall. Clinical Impression: It was determined that this patient may be appropriate for treatment with LSVT BIG exercises due to his decreased gross motor control and coordination. Intervention: This patient was treated in outpatient physical therapy for eight weeks with LSVT BIG exercises along with balance training, coordination training, aerobic exercise, and patient and family education. Outcomes: The effectiveness of these interventions was measured using the TUG, gait assessment, specific motor coordination testing, and gross motor coordination testing with karaoke stepping. Discussion: The purpose of this case report is to describe a modified LSVT BIG protocol in conjunction with patient education, coordination training, and balance training for a patient living with Huntington’s disease. A multidisciplinary approach, as well as a variety of physical therapy treatments, were necessary for the success of this patient. Further research is necessary to determine if LSVT BIG exercises are effective for treating patients with neurological conditions aside from Parkinson’s disease.
