Background: The purpose of this case report is to analyze barriers to a patient’s rehabilitation in an inpatient setting that ultimately impacted the therapeutic treatment provided as well as the overall patient outcomes. Case Description: Patient XYZ was an 80-year-old male who was transferred to the neuroscience intensive care unit following an admission to the emergency department (ED) due to a head trauma that caused a subdural hematoma. The patient’s length of stay at the hospital was lengthened due to various complications relating to both mobility as well as cognition. Clinical Impression: Various barriers analyzed in this report all contributed negatively to the individual’s prognosis. These barriers all impacted Patient XYZ’s eventual discharge to a skilled nursing facility. Intervention: Physical therapy provided throughout the course of care initially targeted strength with therapeutic exercise due to the patient’s limited ability to participate. Throughout his time at the acute care hospital, the physical therapy approach transitioned to more functional mobility. Clinical Outcomes: The two primary outcome measures utilized were level of assistance provided and the Boston University AM-PAC “6 Clicks”. Discussion: Barriers to Patient XYZ’s rehabilitation included surgical approach, decreased cognition, medical complications, low frequency of care, age, and lack of social support. Some barriers were modifiable versus non-modifiable, but each were uniquely addressed through physical therapy interventions, with opportunities for improvement upon future interactions with said barriers.
