Background: Adhesive capsulitis of the shoulder is a common musculoskeletal condition causing pain and limited mobility of the shoulder. Manipulation under anesthesia and arthroscopic capsular release are procedures performed to treat adhesive capsulitis when symptoms are not relieved with conservative treatment. There is a lack of clinical practice guidelines for physical therapy care following this procedure. The purpose of this case report is to highlight the rehabilitation management for an adult individual following an arthroscopic capsular release and manipulation. Case Description: The patient is a 42-year-old male presenting to physical therapy one day post arthroscopic capsular release and manipulation of his right shoulder. He presents to physical therapy with pain and impaired range of motion and strength. Intervention: Physical therapy interventions aimed to improve range of motion, neuromuscular control, and strength to promote improved function of the patient’s right arm. Dosing of physical therapy interventions changed over the course of 11 weeks based on the patient’s needs. Outcomes: Outcome measures included QuickDASH Questionnaire, goniometric measurement of passive and active ROM, manual muscle testing, and patient reported pain. Discussion: 11 weeks of physical therapy rehabilitation following arthroscopic capsular release and manipulation resulted in improvements in all outcome measures. The patient returned to his usual activities without limitations or pain. This case outlines the dosage and specific interventions selected throughout rehabilitation following arthroscopic capsular release and manipulation. Discussion is had about potential additions or modifications to the patient’s rehabilitation protocol to further improve outcomes.
