Background: COVID-19 has been a debilitating disease to millions of people, typically resulting in acute respiratory distress syndrome (ARDS) with many associated conditions in various organ systems. As this is a new condition, there is little research regarding physical therapy management of these patients and therefore the purpose of this report is to present a potential physical therapy management plan following COVID-19. Case Description: Patient A was admitted at the opening stages of the SARS-Cov-2 pandemic in April, and had a very long and complicated episode of care. His total episode of care lasted 175 days, with inpatient rehab consisting of 61 days of the total stay. Interventions: The main interventions in this case consisted of task-specific training broken down into various components involving aerobic training, strengthening, and functional mobility. Outcome Measures: The outcome measures utilized in this case are the Functional Independence Measure (FIM), the Inpatient Rehabilitation Facility – Patient Assessment Instrument (IRF-PAI), the Six Minute Walk Test (6MW), and general manual muscle testing (MMT). Discussion: The patient was able to demonstrate functional gains in the FIM (increased by 22 points), the IRF-PAI (increased by 47 points), the 6MW (increased by 460 feet), as well as gross strength gains over the course of 61 days. However, the patient has a poor prognosis for full recovery due to overall lung function and will continue with a follow up with a cardiothoracic surgeon due to the presence of a necrotic right lung.
