Introduction: Veteran patients often have increased comorbidities or psychosocial factors compared to the general population impacting rehabilitation following surgery. In patients undergoing surgery, these may be exacerbated impacting physical therapy treatment and outcomes. The purpose of this case report is to examine how psychosocial factors in a veteran patient after a total knee arthroplasty (TKA) influenced clinical decision making and treatment interventions. Case Description: A 72-year-old Vietnam era male veteran underwent a right TKA due to persistent pain. The veteran had a history of psychosocial factors that were exacerbated post-operatively. Intervention: With an exacerbation in psychosocial factors 3 weeks post-op, the frequency of physical therapy sessions increased. To supplement current physical therapy clinical practice guidelines for TKA rehabilitation, TENS and a flexionator machine were used. Outcomes: The patient demonstrated improvements in ROM and functional ability. The LEFS score did not show clinically significant improvement; instead the KOOS-JR outcome measure should have been used. Patient reported improved psychosocial factors following changes in physical therapy interventions. Discussion: Many factors influence physical therapy outcomes. Further research is needed to determine appropriate treatment interventions specific to the veteran population following a TKA to improve short term outcome measures such as ROM and patient reported functional abilities.
