Background: Hamstring strain injuries (HSI) are extremely common in athletics where the estimated incidence per 1000 hours of exposure is 0.87 in noncontact sports and 0.92-0.96 in contact sports. A previous HSI is the highest predictor of developing a HSI. There are many global factors that put athletes at risk of hamstring strain including, but not limited to: strength imbalance, fatigue, decreased flexibility, and deficient lumbopelvic control. This case study highlights the importance of constant re-evaluation to recognize movement pattern deficits and address them with appropriate and effective interventions. Case Description: This is a 16-year-old male who is a multi-sport athlete reporting to outpatient physical therapy three days following a hamstring strain during sprinting at football practice. The patient reports running sprints when he felt a pop and immediate pain in his distal and lateral right hamstring. He sustained a previous hamstring strain approximately four weeks prior which he went to physical therapy for at a different clinic. Interventions: Physical therapy focused on symptom management in the context of return to sport with methods such as functional dry needling, hip mobility, eccentric exercises, return to run, and return to sport progression. By discharge, the patient was no longer experiencing pain with sprinting or at practice. Outcome Measures: The primary measures considered for the patient were the Selective Functional Movement Assessment, Patient Specific Functional Scale, and comparable signs pre- and post-intervention. Discussion: This case report demonstrates the importance of constant and thorough evaluations of movement patterns and proper selection of interventions both above and below the area of injury for return to sport training.
