Background: Lateropulsion affects 10-18% of individuals after stroke and is associated with longer recovery. Despite its prevalence and negative association with functional outcomes, little consensus exists on intervention strategies to target this midline deficit. Postural training and visual feedback are common intervention approaches to address midline impairments in lateropulsion, but little research has been done in the acute care environment. Purpose: The purpose of this case report is to 1) examine the complex management of an individual with multiple comorbidities post-stroke and 2) to evaluate the efficacy of interventions to address lateropulsion in the acute environment. Case Description: The patient was a 63-year-old male with a right-sided ischemic middle cerebral artery stroke resulting in severe lateropulsion. The patient was admitted to a major medical center and received forty minutes of physical therapy three times per week for two weeks before being transferred to an acute inpatient rehabilitation facility. Intervention: The primary focus of physical therapy was early mobilization and re-orienting to midline positioning using postural and visual feedback training. Outcome Measures: The Inpatient Basic Mobility Assessment and Gait and Mobility Assessment were used as primary outcome measures of functional mobility. Discussion: Postural and visual feedback training were used to reorient an individual with post-stroke lateropulsion to vertical midline. Though this individual exhibited little retention of postural retraining, significant within-session improvement of vertical midline was seen with the use of a mirror for visual feedback. This case critically examines the usage of mirrors as visual feedback in a patient exhibiting lateropulsion in the acute environment and discusses prognostic factors affecting the efficacy of interventions.
