Background and purpose: Achilles tendonitis is a common injury in those who participate in large volumes of running, jumping, or sports that integrate both. It can be caused by repetitive stress or strain on the Achilles tendon, a band of fascial tissue that originates at your calf muscles and inserts into your calcaneus. The purpose of this case report is to describe the presentation of a 28-year-old male with Achilles tendonitis who was treated with a progressive strengthening program consisting of concentrics, eccentrics, and plyometrics, with intent for return to competitive triathlon. Case Description: A 28-year-old male triathlete presented with bilateral Achilles tendon pain of approximately one month. The patient’s primary complaints were pain in the midportion Achilles following hill sprints that were newly added to his workout regimen. The pain soon progressed to being present with any amount of running he did. Examination: Palpation to the midportion Achilles was tender, in addition to stretching into dorsiflexion being mildly tender. Manual muscle testing of plantar flexion was weak and mildly irritable for his symptoms. Palpation of the retrocalcaneal bursitis was not painful and neither was stretching or contraction of the posterior tibialis tendon. Discussion: This patient was able to slowly progress back into running after five weeks of treatment that consisted of isometrics, followed by concentrics, eccentrics, and lastly ending with plyometrics.
