Background: Social Determinants of Health (SDOH) are factors outside of the health sector, but many studies have shown that they play a significant role in population health. In fact, many studies have shown that 30-55% of all health outcomes are related to these factors; however, they are often overlooked by healthcare professionals. Purpose: Using an example of a patient with a complicated social history, the purpose of this case report is to illustrate the addition of SDOH into discharge planning to address the patient’s unique needs. Case Description: This patient was a 52-year-old female admitted to an inpatient rehabilitation facility after having a stroke. Decision making surrounding intervention and discharge recommendations for this patient was complex due to her social conditions. Discussion: This case report highlights how the interdisciplinary team incorporated the Social Determinants of Health into discharge planning for this patient. In doing so, the rehabilitation team was able to provide the patient with relevant intervention and resources to aide navigation of her environment after discharge.
