A Case of Small Sized Optic Nerve Sheath Schwannoma Presented with Rapidly Worsening Visual Acuity

Abstract

Schwannomas are benign tumors arising from Schwann cells in the peripheral nervous system, which account for approximately 10.4% of all primary intracranial tumors. The most frequently affected nerve is the vestibular nerve, followed by the trigeminal nerve. Although schwannomas of other cranial nerves are very rare, all have myelinated sheaths composed of Schwann cells and are potential sites for intracranial schwannomas. Some authors suggested that the term schwannoma should not be applied to cases of optic nerve origin, but, to our knowledge, two cases have been reported as optic nerve sheath schwannoma to date. Here we report a case of optic nerve sheath schwannoma. A 67-year-old female was admitted for a visual field defect of the right eye. Magnetic resonance (MR) imaging with fat suppression technique demonstrated a 5 mm, oval, homogeneously enhancing mass at the infero-medial side in the right optic canal. The mass was Tl-iso and T2~high intensity on the plain MRI. No other lesion was detected or suggested by neurological and radiological examinations. The lesion was removed through right front-temporal craniotomy combined with anterior clinoidectomy and unroofing of the ispilateral optic canal. Visual envoked potential was monitored during the procedures, which improved immediately after decompression of the optic nerve by tumor resection. Histological examination revealed that the lesion was a schwannoma. The patient had a satisfactory postoperative course, her right visual field recovered completely.
