Abstract

The splenic artery aneurysm is a rare entity that in general does not present symptoms. Although its etiology is unknown, it is frequently associated to many clinic conditions, such as multiple pregnancy, portal hypertension and splenomegaly, systemic arterial hypertension, atherosclerosis, collagen diseases, trauma, and many others. Diagnosis is often done by chance in imaging exams. The relevance of early diagnosis lies on the high mortality seen in ruptured cases. Planed surgical treatment is indicated for aneurysms larger than two centimeters, or in cases of fast growth seen in clinical monitoring, asymptomatic aneurysms, or aneurysms associated to pancreatitis and pancreatic pseudocyst. This report presents a patient in whom two splenic artery aneurysms were identified during the investigation of a pulsating abdominal mass. The patient had portal hypertension caused by schistosomiasis. Ultrasound, abdominal CT scan, and selective arteriography of the splenic artery contributed to the diagnosis. During the surgical procedure two saccular aneurysms were found with approximately six and ten centimeters. The patient was then submitted to aneurysm removal with splenectomy and azygo-portal disconnection.
