Barriers and boosters of adolescent mental health recovery: The dual roles of positive and adverse childhood experiences

Abstract
Adverse Childhood Experiences (ACEs) and Positive Childhood Experiences (PCEs) play critical roles in adolescent mental health, influencing onset and recovery. Informed by the Social-Ecological Risk and Protective Factors framework, the current study examines ACEs as barriers and PCEs as facilitators in recovery from anxiety and depression. We used continuous score and categorical subgroup models to capture both linear and potential threshold effects.
Drawing upon the 2020-2021 National Survey of Children's Health data (n = 33,705; ages 12-17), we focused on adolescent sample with lifetime anxiety (n = 6665) or depression (n = 4086). We employed stepwise logistic regression models to test cumulative ACEs and PCEs as continuous predictors and classified them into Low, Middle, and High ACEs/PCEs groups, controlling for sociodemographic covariates.
In the continuous score model, higher ACE scores predicted lower recovery odds, while higher PCE scores predicted greater recovery odds for both conditions, after controlling for ACEs and covariates. Categorical subgroup models showed threshold effects for anxiety: High ACEs were associated with significantly lower recovery compared to Low ACEs, whereas Middle ACEs did not differ significantly. For depression, both Middle and High ACEs significantly reduced recovery. In contrast, Middle and High PCEs consistently predicted higher recovery odds.
Overall, our findings indicate that ACEs serve as barriers and PCEs as promotive resources in adolescents' recovery from anxiety and depression, with linear effects across both conditions and a threshold effect only for anxiety. Interventions fostering PCEs through resilience-building and supportive environments may facilitate recovery.
