Abstract
Because of the greater margin of safety, the disposable left-sided DLT should be the tube of choice for the majority of cases in which lung separation is required. A right-sided DLT should be an alternative when a contraindication to placement of a left-sided DLT exists. Based on clinical reports. Univents or Arndt blockers may be the tubes of choice for patients with difficult airways who require lung isolation or when selective lobar blockade is needed. The Arndt blocker is suitable when nasotracheal intubation is the only choice to secure the airway during OLV. For confirmation of DLTs, Univents, or Arndt blockers, the method of choice must be fiberoptic bronchoscopy, both before and after patient positioning and whenever a malposition of these devices occurs.
