Developing Healthy Habits: A Faith-Based Interdisciplinary Action Framework

Abstract
This interdisciplinary action framework to influence health and well-being enables wholeness within a faith-based community. The framework provides health care professionals and practicum students the opportunity to serve while reducing negative health outcomes and social inequities, by integrating the elements of what is important to individuals. This article illustrates how innovative methods and collaborations are instrumental in stimulating one faith-based bilingual community to engage in healthy habits such as walking, developing a vegetable garden, and using a self-care transition roach.
