EFFECT OF VERAPAMIL ON LEFT-VENTRICULAR PERFORMANCE IN CONSCIOUS DOGS

Abstract
The effect on left ventricular function of verapamil, a drug which reduces Ca influx across the cell membrane of myocardial cells, was studied in 13 conscious dogs. Each dog received i.v. bolus doses of 0.125, 0.250 and 0.375 mg/kg of verapamil while measurements were made of cardiac output, left ventricular pressure and its maximal 1st derivative (dP/dtmax) [change in pressure with time], left ventricular internal diameter and aortic pressure. Maximal effects included tachycardia (+64%), decreased stroke volume (-38%), decreased mean aortic pressure (-14%), decreased total peripheral resistance (-35%) and decreased dP/dtmax (-32%). Left ventricular diameter increased at end-systole. After propranolol, tachycardia was less at the lower 2 doses of verapamil and there was no change in heart rate at the highest dose; the decreases in dP/dtmax and aortic pressure were accentuated. After propranolol plus atropine, bradycardia occurred at all doses, stroke volume was unchanged and maximal changes included -52% in dP/dtmax and -32% in mean arterial pressure. Verapamil in dosages used clinically exerts opposing direct and indirect effects on cardiac pacemakers and is a potent myocardial depressant and vasodilator. The depressant effect of verapamil on the myocardium is accentuated by .beta.-adrenergic blockade.
