Abstract
The Community Care Network (CCN) vision is a helpful tool for orientating health systems toward the health of populations and communities. Evolving community care networks are guided by the following 5 goals: 1. increasing access and coverage, 2. enhancing the accountability to the community, 3. imbuing the health care system with a community health focus, 5. using health care resources more effeciently. By working toward these goals simultaneously, changing systems learn how to best serve populations and communities in cooperation with other local resources.
