•Immunohistochemistry can be helpful in identifying specific subtypes of struma ovarii.•A multidisciplinary team approach is warranted for rare tumor types such as struma ovarii.•Thyroidectomy can be considered for patients with malignant struma ovarii with papillary carcinoma.
