Marked secondary iron overload due to iron infusion, a case report

Abstract
The patient was a 75-year-old woman. At age 65, she underwent distal gastrectomy for gastric cancer. She was referred by a nearby doctor for a low-grade fever and liver dysfunction. On her visit, she had elevated hepatobiliary enzymes. Viral hepatitis and autoimmune hepatitis were ruled out. Plain CT revealed significantly elevated CT values of the liver and spleen, and MRI showed marked hypointense in both T1W and T2W, suggesting siderosis. There were no findings suggestive of iron deposition in other organs. Serum iron was as high as 210 μg/ dl , and serum ferritin was elevated to 6600 ng/ ml . In the biopsied liver tissue, marked iron deposition was observed not only in Kupffer's cells but also in hepatocytes. The patient had no history of blood transfusion, but had a history of intermittent intravenous iron injections for about 30 months for iron deficiency anemia after surgery for gastric cancer. A diagnosis of secondary iron overload (secondary hemochromatosis) was made, and an intramuscular injection of 500 mg of deferoxamine was started . This is a pathological condition that requires careful attention when continuing intravenous iron infusions.
