Organizational support and burnout among gender-affirming care providers: Evidence from an exploratory factor and structural equation model

Abstract

Gender-affirming care (GAC) providers experience distinct occupational stressors associated with caring for a historically marginalized population amid escalating sociopolitical hostility and legal scrutiny. Despite growing concern about provider burnout, limited research has examined how organizational support shapes well-being and workforce retention. This study examined associations between organizational support, provider well-being, and turnover intention among GAC providers. This cross-sectional study analyzed 2024 survey data from N = 142 U.S. GAC providers. Well-being was assessed using validated measures of burnout, compassion satisfaction, psychological distress, coping, and intention to leave. Organizational support items assessed autonomy, supervisor support, and workplace climate. Exploratory factor analysis (EFA) identified organizational support domains, followed by structural equation modeling (SEM) to test relationships among study variables. EFA supported a three-factor organizational support model. Higher perceived organizational support was associated with lower burnout and psychological distress and higher coping and compassion satisfaction. Burnout was associated with greater psychological distress and higher intention to leave and mediated associations between organizational support and these outcomes. Individual coping showed marginal associations. Organizational support plays a central role in sustaining the well-being and retention of GAC providers and future interventions should address organizational conditions that shape provider well-being.
