Osteoid osteoma of the fifth metatarsal: a case report and literature review

Abstract
This is a case report of a 24-year-old female with a 4-year history of pain in the lateral aspect of her left forefoot. The onset was atraumatic, and the condition was refractory to multiple conservative measures including orthotics and injections. A classic history of pain, which was worse at night and relieved with non-steroidals, in combination with plain radiographs revealing a lytic lesion prompted appropriate diagnostic testing. This allowed accurate diagnosis and treatment with a burr-down technique, which yielded complete relief of the patient's presenting symptoms. This case demonstrates the atypical plain radiographic findings common to intra-articular osteoid osteomas and emphasises that a high index of suspicion should be maintained for lesions such as osteoid osteoma when the presentation is that of atraumatic pain that is worse at night and relieved by non-steroidals. A succinct review of the literature is also provided.
