Radical Spinal Cordectomy for Conus Glioblastoma: Technical Case Instruction: 2-Dimensional Operative Video

Abstract
Diffuse midline gliomas, particularly the H3 K27M-mutant (infiltrating astrocytoma, World Health Organization Grade IV), are rare and aggressive tumors of the central nervous system.1,2 We present the case of a 53-year-old man who presented with progressively worsening lower extremity dysfunction, including bilateral foot weakness, urinary frequency, areflexia, erectile dysfunction, and spastic contractions. Given the irreversible nature of the patient's neurological deficits and the poor prognosis associated with conventional treatments for this type of tumor, radical cordectomy was chosen.3-6
Patient consent was obtained, and the patient underwent a T6/T7 laminectomy with T8, T9, redo T10-T12, and L1-L5 left hemilaminectomy and T6 cordectomy with intraoperative confirmation of negative proximal margins and en-bloc resection of the spinal cord with nerve roots. Postoperatively, the patient demonstrated expected T6 paraplegia with associated neurological deficits but no significant autonomic dysfunction. At 24 months postoperatively, an MRI revealed no signs of recurrence. The successful outcome underscores the importance of individualized treatment planning and meticulous surgical technique in managing this challenging condition. IRB approval was not required, and the patient consented to the publication of the images and video.
