Abstract

•PML is a demyelinating disease caused by reactivated infection with JC virus.•Prolonged or profound states of immunosuppression are the triggering factors of PML.•PML in patients without obvious immunosuppression can also be encountered.•Radiological “milky way sign” can raise the suspicion of early PML changes.•The prognosis of PML is unfavorable if immunocompromise cannot be corrected.
