Single-Port Robot-Assisted Laparoscopic Radical Cystectomy with Intracorporeal Ileal Conduit

Abstract
Introduction: Robot-assisted radical cystectomy (RARC) has been shown to provide perioperative benefits associated with minimally invasive surgery. The advent of the da Vinci® Single-Port (SP) platform (Intuitive Surgical, Inc., Sunnyvale, CA, USA) allows RARC to become even less invasive than previous conventional MP robotic approach. The SP platform provides the surgeon access to three multijointed robotic arms and a multijointed camera all of which can fit through a single 25 mm channel. We present our technique utilizing the SP platform for RARC and review our experience with this procedure.
Materials and Methods: After Institutional Review Board approval was granted, we reviewed the medical records of patients who underwent RARC from February 2020 to November 2020 performed by a single surgeon at our institution. A total of 13 patients who underwent SP RARC were identified.
Results: Of the 13 patients who underwent SP RARC, mean age was 64 years with mean body mass index of 27 kg/m2. Mean operative time was 387 minutes (interquartile range [IQR] 332–405) with estimated blood loss of 119 mL (IQR 100–150). Although all diversions were created with an intracorporeal approach, the majority of patients underwent ileal conduit creation (n = 11). Mean length of stay was 5.3 days (IQR 4–6). Only two patients reported Clavien–Dindo Grade III or greater postoperative complications with two patients returning for percutaneous abscess drainage. No intraoperative complications occurred.
Conclusion: Utilization of SP platform for RARC is safe and effective. This surgical modality can be offered to properly selected patients.
