Abstract
Vertebral artery origin stenosis (VAOS) is a treatable cause of posterior circulation stroke in 9-20% of cases. VAOS can cause stroke due to artery-artery thromboembolism or flow-related posterior circulation hypoperfusion. It is recommended to maximize medical treatment of asymptomatic VAOS. Endovascular treatment of symptomatic VAOS is technically safe and effective, and may be used in secondary stroke prevention in light of the low complication rate of the procedure. The major concern with endovascular treatment appears to be long-term durability owing to significant restenosis rates.
