Abstract

This case report documents the prosthodontic treatment of a 43-year-old male with special care needs who presented with a 13-year history of complete edentulism and prosthodontic diagnostic index (PDI) type IV. The patient was unable to tolerate his previous dentures. The purpose of this report is to highlight the advantages of intraoral scanning and a modifiable milled trial denture in the care of completely edentulous patient requiring special care. The treament sequence selected for this patient included (1) intraoral scan to fabricate custom trays (2) border molded PVS master impressions (3) completed denture bases and wax occlusion rims (4) milled polymethylmethacrylate (PMMA) trial dentures (5) injection-processed PMMA gingiva and incorporation of posterior denture teeth onto trial dentures, and (6) follow-up appointments. The patient reported he could wear this new set of dentures and was successful in learning to eat with them. However, the prognosis remains guarded due to PDI classification and advanced mandibular resorption.
