Treatment of the Patients with Asymptomatic Intracranial Meningioma

Abstract

The aim of this study is to consider the surgical indication of patients with asymptomatic intracranial meningioma. We operated on 28 patients with intracranial meningiomas from 1992 to 1996. Surgical outcome and clinical features for the patients with asymptomatic meningiomas (10 cases) were compared to patients with symptomatic meningiomas (18 cases). There was no surgical mortality and morbidity among the patients with asymptomatic meningiomas. Among the patients with symptomatic meningiomas, radiological features were 1) large size: 2) no calcification; 3) low intensity on MRI T1- weighted image, and: 4) high intensity on MRI T2-weighted image. Postoperative neurological deterioration was observed in 5 cases. These deteriorations were caused by injury of the arteries involved in the tumor in 4 cases, and injury of the motor cortex in 1 case. It is suggested in this study that surgical treatment is recommended even in patients with asymptomatic meningioma when the tumor has clinical features of symptomatic meningiomas or tend to involve cerebral arteries and is located near the motor area.
