Why Cycloplegic Refraction is Still Essential in Some Adults with Diplopia

Abstract
Cycloplegic refraction is an indispensable tool in the evaluation of adult diplopia. It is necessary for accurate assessment of the accommodative component of strabismus, and often reveals issues of anisometropia and astigmatism that are not uncovered without it. A set of three yes/no questions is introduced as the "Diplopia 3-Step Test." The questions are designed to avoid common pitfalls of diplopia evaluation. The role of cycloplegic retinoscopy in diagnosis and treatment under each "Diplopia 3-Step Test" scenario is discussed.
