To the Editor:
With regard to the Diuretic Comparison Project by Ishani et al. (Dec. 29 issue),

the authors found that the incidence of hypokalemia was higher in the chlorthalidone group than in the hydrochlorothiazide group (6.0% vs. 4.4%, P<0.001). This finding is not surprising, since, as emphasized in the accompanying editorial,

chlorthalidone has a longer half-life and is 1.5 to 2 times as potent as hydrochlorothiazide. However, a careful reanalysis of the chlorthalidone-based Systolic Hypertension in the Elderly Program (SHEP) trial

and the editorial by Schrier about that trial

showed that after multiple adjustments, participants in the active-treatment group . . .
