Abstract
The Music Therapy Support Group--Assessment Form (MTSG-AF; Dvorak, 2015) is a measure of cancer patients' or family member/friends' music preferences, interest in group sessions, and expectations for music therapy support groups. The MTSG-AF consists of the following sections: Demographic information, information on cancer diagnosis (5 items), information about family member or friend (2 items), and music preferences and experiences (8 items). Responses for each item (e.g., "Please indicate your experiences with music in your life by checking each experience that applies") are open-ended, forced-choice, and multiple-choice. Psychometric information was not available at the time of test development.
