This report presents the comments from surveys sent to parents after their children were enrolled in the hawk-i program for one year. These comments were received between July 1, 2002, and June 30, 2003. The comments have been grouped by topic area and health plan to provide a more complete picture of respondents views of the hawk-i program. Readers who are interested in the results of surveys used in the evaluation of this program should refer to the report hawk-i Impact on Access and Health Status (Fourth Evaluation Report), Public Policy Center, April 2004; http://ppc.uiowa.edu/hawk-i/. The evaluation of the hawk-i program was sponsored by the Iowa Department of Human Services at the direction of the hawk-i Clinical Advisory Committee and Board of Directors and the Iowa Legislature. The survey process (mailing, telephone follow-up calls, and data entry) was completed by MAXIMUS, the fiscal intermediary for the program. Comments were prepared for the production of this report by researchers at the University oflowa Public Policy Center. The comments do not express the opinions of the Iowa Department of Human Services, the hawk-i program, or the University oflowa Public Policy Center. This project was not sponsored or conducted by the individual health plans providing services to hawk-i enrollees.
