This paper describes five community pharmacy practice innovations using lessons and quotes taken from pharmacists who have implemented the innovations. The 5 practice innovations include Cost Plus Pharmacy, Long-term Care at Home, Functional Medicine, Direct Contracting and Direct Patient Services under a Team Model. For each of the innovations a guide is provided to support implementation in community pharmacies.
