Abstract
Living donor transplantation is the preferred treatment for end-stage kidney disease (ESKD). While living donor transplantation generally substantially enhances the quality of life and prolongs survival of the kidney recipient, it is now known that living donation is associated with a small but significant increase in the risk of ESKD in the donor, with heightened risk when there is a family history of kidney disease. The increased risk of kidney failure in biologically related donors may reflect the shared inheritance of a genetic variant causal for monogenic renal disease or the sharing of one or more renal disease susceptibility or risk alleles. In this chapter, we discuss the importance of establishing a cause of ESKD in the recipient candidate and describe tools for the evaluation and exclusion of several common genetic diseases in the living donor candidate. We also describe the role of apolipoprotein L1 (APOL1) renal risk variants and sickle cell trait as important risk factors for advanced kidney disease and offer a perspective on testing for these genetic traits in living donor candidates of susceptible ancestry.
